We ask that all
new patients joining the practice, take care of the charges in full on
their initial visit, regardless of the insurance involvement.
You must assume the
responsibility of paying for all services rendered. Your insurance
benefits may assist you in paying for our services; however, you are
personally responsible for the fees incurred during treatment.
Your insurance coverage
may or may not include benefits for services provided in this office.
The benefits you may receive are established by the terms of the contract
that has been purchased by your employer or you. No two contracts
are identical. Very few cover all of your expenses. The
formula your insurance carrier uses, if benefits are covered, may be less
than the fee we have established as necessary to conduct our business.
It is common for a plan to
pay a percentage of what the insurance industry calls the usual, customary
or prevailing fee. Each insurance carrier establishes their usual,
customary, and prevailing fees by use of a complicated formula which is
confidential and not made public.
Many policies require that
an annual deductive be paid.
Major
credit cards such as Visa, MasterCard, Discover, and American Express are
all accepted in our office.
